The
following information is provided to aid new patients
in the scheduling process, and should answer most
questions you would have when scheduling an appointment.

Verify that Ramadan Hand Institute is a
provider under your insurance. If you are
unsure, please call our office or your insurance
company. If we are not a provider and you
would still like to be treated at Ramadan
Hand Institute, prior authorization is required
from your insurance company.

When calling to schedule your appointment,
you will be asked some pertinent questions,
which will help us in scheduling your visit.
Please also have your insurance card available
if insured. This allows us time to verify
and get authorization on insurance if it is
required.

On the day of your first appointment, please
bring any X-Rays, MRIs, EMGs or tests done
prior to your visit that relate to the injury
or disorder for which you are being seen.
You will also need to bring in a list of your
current medications, form of ID (driver’s
license or state ID card) and your insurance
card. Be advised that new patient appointments
normally take longer than those for established
patients. It can take up to an hour or hour
and a half, depending on the injury or disorder.
Most new patient appointments are scheduled
for the afternoon, but morning appointments
can be arranged.

If you are being seen for a 2nd opinion
or IME (Independent Medical Evaluation), please
have all medical records with you at the time
of your appointment, or have them sent to
the Lake Butler office prior to your arrival.
